The Summary Care Record
What is the Summary Care Record? The Summary Care Record is an electronic summary of a patient’s key health information (medication, allergies and significant medical history) that will be held at national level so that NHS organisations treating that patient anywhere in England can access the record – with the patient’s verbal agreement – to support the treatment being provided. The Summary Care Record (SCR) has been trialled in other parts of the country and from 12 March 2010 NHS Wirral are sending information packs out to all patients registered with Wirral Practices so that they can consider this and opt out if they wish to. What do I have to do about this? If you are happy to have a record you need do nothing. If you wish to opt out you will need to complete an Opt-out form and return this to your GP Practice before 30 June 2010. Your records will then be coded so no upload goes to the National computer system. There is no opt-out form in the information you will receive. However it is available at the following link http://www.connectingforhealth.nhs.uk/systemsandservices/scr/documents/noscr.pdf Alternatively we have copies of the Opt-Out form here at the surgery that you can collect. For reasons of cost we cannot send the form out to patients who request it – unless you send in a stamped, self-addressed envelope.
