NEWS We can offer medicals to patients who are not registered with our practice Upton Group Practice is able to offer medicals for the following to any member of the public who requires such:- * Pre-employment * Medicals for third parties * HGV medical * PSV medical * Taxi driver medical * Elderly driver medical * Racing driver medical * Diabetic driver medical * Sports medical eg. Diver, gliding etc. * Visa application medical * Oil rigs fitness * Holiday cancellation examination and report * Seat belt exemption examination and report * Fitness to travel examination and report The appointment for such a medical is half an hour in length and the cost of these medicals is £98. Payment is required at the time of making an appointment for the medical with the practice and a receipt can be provided. This fee is non-refundable if the patient fails to attend the appointment or cancels at less than 2 hours notice. The patient is responsible for bringing the necessary paperwork for completion to the appointment. If you wish to arrange a medical for any of the above please call in to the practice or contact our Reception staff on 0151 677 0486. Please advise when making the booking that you are not registered with the practice.
Farewell and welcome
Patients out of contact with the practice for over 3 years will be removed from the practice list Please be aware that Wirral PCT is working with Central Operations Mersey to contact male patients on our practice list between the ages of 25 and 50 who have not had contact any with our practice for 3 years. If you do not respond to their letter (asking you to confirm that you are still resident at the address we have for you and that you wish to remain registered with our practice) they will remove you from our practice list. This will mean that your records will be returned from the practice to Central Operations Mersey and you will not be registered with any GP. Should you need medical care in the future you will need to complete registration forms with our practice to re-register as our patient and it will take up to six weeks for us to recall your medical records from Central Operations Mersey. Whilst we are happy to re-register patients who are in our practice catchment area the need for this can be avoided if you would please respond to any letters you receive from Central Operations Mersey regarding your registration remaining current with our practice. Thank you.
Appointments and Repeat Prescriptions On-line We now have the facility to offer patients the choice of booking appointments and ordering repeat prescriptions on-line. If you are interested in signing up for this service please ask at Reception next time you are in the practice. You will be provided with a unique login user name and password. You can then book, view and cancel your appointments on-line at any time - even when the practice is closed. You will also be able to make requests for repeat prescriptions - although it still takes 48 hours from the time the practice is next open for your request to be processed and your prescription to be ready for collection. If you wish your nominated pharmacy to collect the prescription from the practice for you it is up to you to let them know when you put a request in to us and to advise them that it will be ready for them to collect from us two working days later. However please note that you will only be able to log in and access the service if you have already obtained your User name and Password by coming in to the practice to sign up. CARDIOVASCULAR DISEASE HEALTHCHECKS At Upton Group Practice we are always looking at ways to improve our patients' health. At present we are calling forward all patients in the age range 40 to 74 for a health check which will assess your risk of developing heart disease, stroke, kidney disease or diabetes in the future. As part of your screening we are doing a blood test to check your cholesterol and sugar levels. The cholesterol test gives us a breakdown of the different fats including good/bad cholesterol. As we require you to fast overnight for a cholesterol test we need to book these appointments in the morning before 11am. The health assessment will be made by using the information you provide to a healthcheck questionnaire the Nurse will run through with you, together with your blood test results and your blood pressure reading. If your risk of developing any of these conditions is significantly increased, we will call you in for a further appointment with the nurse or doctor to discuss modifying risk factors in order to help you make changes for a healthier future and we will review your progress annually. If your risk assessment is low we won't need to risk assess you again until 2016. Smoking If you would like advice on how to quit smoking please ask at Reception about our Smoking Cessation clinic. Alcohol Current Government Safe Drinking Guidelines are :- Ø 14 to 21 Units per week for females Ø 21 to 28 Units per week for males The Community Programme The Community programme is a range of free health and wellbeing activities designed to help local residents become more active and to lead healthier lifestyles. Activities include Weight Management courses, Chair Based Exercise, Gym sessions, Group Walks, Allotments course, Healthy Cookery, Relaxation, Tai Chi, Yoga and Stop Smoking sessions.
