HEALTH VISITORS

Helen Morgan and Pauline Davies.

Our Health Visitors are based at Ganneys Meadow Early Years Centre and come in to our practice on Monday and Thursday afternoons.

Our Health Visitors can be  contacted directly at Ganneys Meadow on 678 0908  or you can leave a message with our Reception staff for routine matters.

 

DISTRICT NURSES

Rachel Kershaw, Jeff Rodway, Jill Parr, Margaret Dodd, Lynn Mansell, Jane Chamney and Jan Kirktlan

 

We have a team of seven District Nurses (2 work on a jobshare basis) who are based at Eltham Green Clinic  but are in daily contact with the practice.  Our District Nurses look after our patients in the community, visiting those who are housebound but have special nursing requirements.   They attend our patients in their own homes to assess Nursing needs and offer a comprehensive range of skilled Nursing care.  You can contact the District Nurses directly on 604 1383 (please leave a message on their answerphone) or for routine matters you can leave a message with our Reception staff.

 

 

MIDWIVES

The Midwives from the Greenwood Team  work with us and come in to the practice on Tuesday afternoons to run an antenatal clinic.

 

COUNSELLORS

Linda Culkin and Joanne Dickson

Our Counsellors take referrals from our GPs to see our patients who need counselling on issues such as bereavement, depression, stress and relationship problems.    Joanne works with us on Tuesday mornings, whilst Linda is in on Fridays.   Our Counsellors also work closely with the Psychotherapy department at St Catherine’s hospital and if the times that they are available here at the practice are not suitable for a patient or a patient would prefer to see a male Counsellor an appointment can be arranged via the team of Counsellors who work at the hospital.   The waiting list for counselling is usually about twelve weeks for routine cases but if the doctor feels your case is more urgent then arrangements can be made for you to be seen sooner.

 

NURSE VISITORS FOR OLDER PEOPLE

Nuala Browning, Ann and Eileen

 

Nuala, Ann and Eileen are based at Parkfield Medical Centre and their role is to visit our elderly patients to improve their quality of life at home and in partnership with other professionals.    An holistic assessment is made of the physical, social, environmental needs for the elderly person and their carer.    The Visitors for Older People are able to advise and inform their clients and their carers about health issues and Illness and engender a greater sense of self-reliance.   They represent the older person whilst working in close liaison with statutory and voluntary agencies involved in the care of that person.
